Health and pollution

Asbestos OCD

Doubt about building dust and fibres can develop into research, cleaning and attempts to map possible routes of spread. In asbestos OCD, doubt continues after the specific risk has been investigated. Building dates, dust, wind direction and possible contact routes are mapped to determine whether invisible fibres may have spread to clothing, a car, the home or other people. Daily life may gradually become governed by which buildings, materials and traces can be regarded as safe, even when there is no new information about exposure.

Control and order

Checking OCD

Repeated checking may concern external conditions or internal experiences such as thoughts, feelings and memories. In checking OCD, checking may concern doors and appliances or feelings, thoughts, memories and bodily sensations. The check is intended to create certainty but may make the experience less clear and the memory less convincing. Checking may expand from a single action into long sequences because every check creates new questions about whether it was performed or remembered correctly.

Health and contamination

Chemical OCD

Doubt about chemical residues and invisible transfer chains can lead to research, washing, ventilation and disposal. In Chemical OCD, residues, dose and possible exposure routes are investigated far beyond the immediate situation. A cleaning product, battery or distant smell may lead to research, washing and mapping the people and objects to which the substance might have been transferred. The threshold for safety readily shifts because even a highly unlikely transfer can feel unacceptable when the imagined consequence is serious.

Health and contamination

Contamination OCD

Fear of picking up or spreading contamination can lead to washing, barriers, avoidance and strict divisions between clean and contaminated zones. Contamination OCD is broader than fear of germs and handwashing. Doubt may centre on picking up contamination, spreading one's own bodily traces or protecting clean zones. Washing, barriers and rules about touching can then play an increasing part in everyday life. The rules may gradually spread from particular situations to the home, relationships and freedom of movement as the person tries to maintain safety at every step.

Existential and moral themes

Death OCD

Thoughts about death and the afterlife can develop into persistent analysis, checking of mental images and the search for certainty. In death OCD, death, non-existence or a possible afterlife become questions that feel necessary to solve. Mental images, philosophical analysis and research are used to seek certainty about something no one can settle completely. Attempts to find the right explanation may briefly ease the distress while making the question even more central and difficult to leave open.

Food, body and safety

Eating and Food OCD

Food choices may be governed by repeated checks and rules about safety, health, morality or responsibility. In eating and food OCD, food choices, preparation and meals are governed by repeated checks and rules. Ingredients, expiry dates and possible consequences are assessed to create certainty about health, contamination, morality or responsibility. The pattern may restrict variety, spontaneity and shared meals because every decision must first satisfy an increasingly extensive set of safety rules.

Health and contamination

Emotional contamination in OCD

Fear of acquiring traits, identity, mood or misfortune through contact, places, words and associations. In emotional contamination, the fear is not physical infection but acquiring traits, identity, morality, mood or misfortune through contact and associations. People, places, words and objects may therefore be avoided or treated as contaminated. Checking can spread through long chains of association, so that more and more of everyday life becomes associated with what the person is trying to keep at a distance.

Control and order

Exercise OCD

Doubt about how exercise is performed and what it achieves can lead to repetition, measurement and rules that are hard to end. In exercise OCD, training is governed by doubt about performance, effect, bodily signals or future preparedness. Exercises, numbers and bodily sensations are checked, while a rest day may feel like a loss of strength, health or safety. Exercise may become inflexible and difficult to adapt to illness, injury or other needs because deviations are treated as risks that must be compensated for.

Existential and moral themes

Existential OCD

Open questions about reality, identity, life choices, death or meaning can become persistent doubt and mental rituals. In existential OCD, open questions about reality, identity, free will, life choices, death or meaning are treated as problems that require complete certainty. Analysis and information seeking rarely provide a lasting endpoint. The more the question is analysed, the more caveats emerge, and ordinary uncertainty may begin to feel like evidence that something crucial has been missed.

Existential and moral themes

False Memory OCD

Doubt about whether something happened can lead to mental reconstruction, questioning other people and searching for evidence. In False Memory OCD, ordinary gaps in memory are examined repeatedly. Mental reconstruction, questioning other people and checking bodily responses can create new details that then feel like possible evidence of something that may have happened. The uncertainty therefore extends beyond the event to whether the memory, a feeling or the absence of a feeling can be trusted.

Responsibility and harm

Harm OCD

Unwanted thoughts about harm can create doubt about intentions, control and moral character. In Harm OCD, unwanted thoughts, images or impulses about harm are examined for what they might reveal about intention, loss of control or moral character. Checking feelings, mentally reviewing events, neutralising and avoiding situations are intended to prove that the person is not dangerous. The thoughts are unwanted, but the very attempt to control what they mean can make them feel more present and more serious to the person.

Health and illness

Health OCD

Fear of physical or mental illness can lead to body checking, research and repeated reassurance seeking. In health OCD, bodily or mental signs are examined to determine whether they indicate physical or mental illness. Body checking, research and reassurance seeking may settle the question briefly until a new sensation or doubt opens it again. Examinations and answers may clarify the current concern, but when the aim is complete certainty, doubt often moves to a new symptom or explanation.

Responsibility and harm

Hit-and-Run OCD

A bump, a sound or an unclear memory from a drive can trigger doubt about whether someone was hit. In Hit-and-Run OCD, a bump, a sound or a brief glance away from the road can trigger fear of having hit someone. The person may drive back, inspect the car or reconstruct the route, but checking often makes sensory impressions and memories less certain. Another check may bring a moment of certainty while reinforcing the idea that every sound or unclear memory requires further investigation.

Control and order

Just-Right OCD

A person may repeat an action or sound, or adjust a physical sensation, until it briefly feels exactly right. In Just-Right OCD, a movement, sound, thought or touch feels wrong, unfinished or out of balance. The person repeats, corrects or starts again in search of a particular sense of completion that rarely lasts. The action is often less about preventing a specific catastrophe than about removing a persistent physical or mental sense that something remains unfinished.

Existential and moral themes

Magical Thinking OCD

Thoughts, words, numbers or small actions may feel connected with harm and trigger private rules and repetitions. In Magical Thinking OCD, a thought, word, number or small movement can feel connected with something dangerous despite the absence of a realistic cause. Repetition, avoidance and mental neutralisation are used to undo the connection or avert the feared harm. Even when the person recognises that the link is not logical, the sense of responsibility may feel too great to refrain from the ritual and simply wait to see what happens.

Symptoms and mechanisms

Meta-OCD

When OCD itself becomes the theme, diagnosis, symptoms and signs of recovery may be examined repeatedly. In Meta-OCD, OCD itself becomes the subject of doubt. The person may try to decide whether a thought really is an obsession, whether the diagnosis fits or whether recovery is genuine, while analysis and information seeking become new ways of pursuing certainty. Treatment, progress and ordinary fluctuations may then be scrutinised as evidence, rather than leaving room for the person to continue living with uncertainty.

Existential and moral themes

MeToo OCD

Doubt about consent, boundaries or one's behaviour can lead to mental review, excessive control and repeated apologies. In MeToo OCD, fear of having violated someone or crossed a boundary keeps returning. Conversations, looks, touches and messages are replayed, while the person turns to rules, witnesses and other people's assessments in search of a definitive judgement. The overriding aim is to rule out every possible misinterpretation of the past, even though new perspectives and moral nuances make such certainty impossible.

Moral OCD

Doubt about guilt, intentions and character can lead to mental review, confession and reassurance seeking. In Moral OCD, doubt about guilt, responsibility, intentions and character keeps returning. Mental review, confession, checking feelings and comparisons are intended to establish whether the person is good enough, but rarely provide lasting clarity. Everyday mistakes and ambiguous situations may become moral tests in which even minor caveats feel decisive to the person's sense of self.

OCD-Related Doubt About Gender Identity

Doubt about gender identity can lead to repeated checking of feelings, comparisons and analysis. When OCD centres on doubt about gender identity, feelings, memories, bodily reactions and gender expression are examined in search of a definitive answer. Comparisons and repeated checks are meant to provide certainty but can make identity feel less clear. The theme can occur in people of any gender identity; the central issue is the demand for certainty and repeated checking, not the answer the person reaches.

Control and order

Optimisation OCD

The demand to make the best possible choice can lead to research, comparison and decisions that never feel settled. In Optimisation OCD, even good solutions keep being measured against the possibility of something better. Research, rankings and calculations are intended to prevent waste or regret, but can delay a decision and reopen choices that seemed settled. The pattern may affect purchases, work, health and the use of time because almost any decision can be measured against alternatives that have yet to be explored.

Relationships and sexuality

Paedophilia OCD (P-OCD)

Unwanted sexual obsessions about children can lead to shame, bodily checking, mental review and avoidance. In P-OCD, unwanted thoughts, memories and bodily sensations are examined as possible evidence of sexual interest in children. Bodily checking, mental review, research and avoidance are intended to disprove the fear, but can make the doubt more prominent. The thoughts are unwanted and conflict with the person's values, but trying to prove this with complete certainty can keep attention fixed on the theme.

Control and order

Perfectionism OCD

Doubt about mistakes and quality can lead to repeated checking, corrections, overpreparation and postponement. In Perfectionism OCD, it becomes difficult to decide when a task is good enough. Repeated reading, corrections and overpreparation are intended to remove every possible error, even when the result already serves its purpose and no longer improves. The time involved can grow considerably because every improvement reveals another possible flaw, so finishing feels like accepting a risk.

Control and order

Permanence OCD

When a choice feels final, doubt can lead to repeated checking, postponement and attempts to keep options open. In Permanence OCD, the transition from provisional to finished becomes particularly difficult. Measuring, documenting and temporary solutions are intended to prevent an irreversible mistake, while decisions are postponed or reopened to prevent later doubt. The pattern may affect purchases, interior choices, tattoos, documents or other choices where the possibility of making changes later feels inadequate.

Family and the postpartum period

Postpartum OCD

Intrusive thoughts about a baby's safety can lead to repeated checking, avoidance and reassurance seeking. Postpartum OCD can focus on the very area in which care and responsibility feel greatest. Intrusive thoughts about harm, responsibility or contamination may lead to checking the baby, avoiding care tasks and repeated attempts to obtain complete certainty. The pattern can create distance from the situations in which the person most wants to be present, even though the thoughts do not indicate wishes or intentions.

Control and order

Pure C in OCD

In Pure C, repetition is prominent, while the underlying drive may be experienced as a bodily urge, sensory discomfort or incompleteness. In Pure C, a visible or mental compulsion does not necessarily arise from a clearly formulated fear. Repetition, evening up and correction may instead be used to alter a bodily urge, sensory discomfort or sense of incompleteness. The aim is often to make a sensation feel completely settled, so the same action may continue even when the person expects no external consequence.

Mental compulsions

Pure O in OCD

In Pure O, compulsions often take the form of hidden analysis, mental checks or internal reassurance. Pure O describes OCD in which intrusive thoughts are prominent while compulsions occur mainly as mental actions. Rumination, internal reassurance, checking feelings and mental review may be almost invisible to others while sustaining doubt. The absence of obvious rituals therefore does not mean that compulsions are absent; the effort to achieve certainty simply takes place in the mind.

Existential and moral themes

Real Event OCD

When guilt and responsibility must be determined with complete certainty, an event may be reviewed repeatedly. In Real Event OCD, the person returns to an actual event to determine motive, guilt and character. Mental review, confessions and self-punishment are intended to produce a definitive moral judgement, but each answer tends to give rise to another doubt. Because the event is real, doubt may seem more legitimate, while the need for complete resolution still follows the repetitive pattern of OCD.

Relationships and sexuality

Relationship OCD

Love, attraction and a partner's qualities may be examined repeatedly to determine whether the relationship is right. In Relationship OCD, doubt about love, a partner's qualities or the relationship's future is answered with feeling checks, comparisons and mental review. Checking is meant to determine whether the relationship is right, but often makes feelings less clear. This may lead to distance, repeated conversations or new tests of the relationship, even though the aim is to protect it from a wrong decision.

Existential and moral themes

Religious OCD

Religious and non-religious people can become caught in doubt about sin, punishment, prayer or religious questions. Religious OCD concerns the content of doubt, not whether the person is religious. Faith, sin, prayer and spiritual consequences may be checked repeatedly through repetition, mental neutralisation and seeking religious certainty. Rituals may resemble ordinary religious practice but are driven by a demand to perform or understand the practice in exactly the right way and without moral risk.

Mental compulsions

Rumination OCD

When thinking is meant to remove doubt, the same question may be analysed again and again. In rumination OCD, analysis, mental review and feeling checks are used to seek certainty about a thought, feeling or event. The answer rarely lasts, because a new caveat or an overlooked detail soon reopens the investigation. It may feel like problem-solving, but the analysis has no clear criterion for when enough thinking has been done, so it continues.

Saving and Discarding OCD

Doubt, responsibility or emotional significance can make discarding difficult even when the person does not want to keep the item. In Saving and Discarding OCD, objects, papers and digital files may be kept to prevent guilt, loss, contamination or an irreversible mistake. Repeated sorting, photographing and reconsidering are meant to make discarding safe, but keep the decision open. The defining issue is not the number of possessions, but that discarding feels like a decision carrying great responsibility and must wait until all doubt has been removed.

Mental illness and control

Schizophrenia OCD

Fear of psychosis or schizophrenia can lead to persistent checking of thoughts, sensory experiences and one's sense of reality. In schizophrenia OCD, thoughts, sensory experiences and one's sense of reality are monitored for signs of psychosis. Background sounds, unusual thoughts or brief moments of unreality are tested repeatedly, and the checking can make experiences feel more unfamiliar. The fear often concerns the possibility of losing control rather than a fixed belief, but repeated tests may increase the sense of unfamiliarity.

Control, attention and self-observation

Self-Monitoring OCD

Thoughts, feelings, voice and body language may be monitored to determine whether the person seems natural and genuine. In self-monitoring OCD, attention shifts from the situation to the person's own thoughts, feelings, voice or body language. Continuous checking is meant to ensure that the response appears natural and genuine, but makes it harder to remain present. Conversations and social situations may then feel artificial because attention continually shifts between taking part and assessing one's own performance.

Guilt, responsibility and self-punishment

Self-Punishment OCD

Guilt and doubt can lead to rules about giving up pleasure, rest, food or company to demonstrate sufficient remorse. In self-punishment OCD, pleasure, rest or care become conditional on feeling good enough or sufficiently remorseful. Deprivation, self-criticism and unnecessarily harsh rules are used to restore a moral balance, but the question of punishment returns. Even after deprivation, doubt may return as a question of whether the punishment was severe enough, sincere enough or may ever be allowed to end.

Body, senses and attention

Sensorimotor OCD

Breathing, swallowing, blinking or other automatic bodily processes may become the focus of persistent attention. In sensorimotor OCD, breathing, swallowing, blinking or other automatic bodily processes become difficult to let recede into the background. The person monitors, controls or tests the process to remove it from awareness, which often makes it more noticeable. Paradoxically, trying not to notice the process requires the person to monitor it, so attention remains fixed on it.

Relationships and sexuality

Sexual obsessions in OCD

When a sexual thought or bodily sensation is examined as possible evidence of wishes or intentions. Sexual obsessions may concern assault, incest, boundaries or inappropriate reactions. Bodily sensations, gaze direction and past situations are checked as possible evidence, while mental review and avoidance maintain the doubt. The content is unwanted, and checking is intended to protect others and the person's sense of self, but it also makes the monitored reactions more prominent.

Relationships and sexuality

Sexual Orientation OCD

When attraction, memories and bodily reactions are checked to gain certainty about sexual orientation. In sexual orientation OCD, attraction, memories and bodily reactions are checked to gain certainty about orientation. Comparisons, tests and reviews of the past may make doubt more prominent regardless of the person's orientation. The theme can affect people of any orientation; the problem is not a particular orientation, but the demand for an answer without any remaining doubt.

Health, strain and control

Stress OCD

Signs of stress and recovery may be monitored through measurements, daily plans and repeated assessments of strain. In stress OCD, sleep, heart rate, concentration and mood become possible warnings of burnout or collapse. Measurements, optimisation and repeated assessments are used to prevent stress, but ordinary fluctuations continually call for another check. Prevention may itself become stressful because rest, activity and work are continually assessed according to whether they increase or reduce a future risk.

Harm, loss of control and self-understanding

Suicidal Obsessions in OCD

Unwanted thoughts or urges about suicide may trigger fear of a hidden wish or future loss of control. With suicidal obsessions, unwanted thoughts, images or urges about suicide are examined as possible signs of a hidden wish or future loss of control. Feeling checks, risk assessments and avoidance are used to determine what the thoughts mean. The thoughts are typically unwanted and frightening, but suicide risk must always be assessed for the individual rather than from the theme label alone.

Control and order

Symmetry OCD

Uneven positions and unequal bodily sensations may trigger arranging, evening up, repetition or counting. In symmetry OCD, an uneven position, unequal bodily sensation or broken sequence may feel intensely wrong. The person arranges, measures, evens up or repeats to create balance until the ritual determines when it is possible to move on. The ritual may be guided by sight, the body or an internal rule and ends only when the sensation fits, rather than when the task is practically complete.

Bodily sensations and tics

Tic-Related OCD

In tic-related OCD, the person has OCD and a current or previous tic disorder. Tic-related OCD means that a person with OCD has, or has previously had, a tic disorder. Tics and compulsions may be clearly separate, affect each other or occur within the same action, without the OCD theme necessarily being about tics. Distinguishing between them therefore requires attention to bodily urges, fear, rules and function, rather than looking only at how the movement appears.

Bodily sensations and tics

Tourettic OCD

Tics, bodily urges and compulsive rules may overlap in actions that must feel exactly right. In Tourettic OCD, a movement or sound has both tic-like and compulsive features. A bodily urge may become bound to rules about force, rhythm, symmetry or precision, so the action must be repeated until it feels exactly right. The pattern may be difficult to distinguish from both tics and classic compulsions because bodily urges and rules about correctness occur together.

Symptoms and mechanisms

x-OCD

Some OCD presentations have a personal or changing theme that does not fit precisely within one named OCD type. x-OCD describes presentations in which the theme is personal, changes or overlaps several named types. The doubt and the visible or mental actions recur, and for some people the search for exactly the right type label becomes a compulsion in itself. The label can be useful when a theme does not fit a fixed category, but treatment still focuses on the function of doubt, avoidance and compulsions.
